Can consuming diet drinks or artificial sweeteners lower sperm count?

Most men worry about heat, tight underwear, or alcohol when they think about fertility. Diet drinks rarely make the list. But the question keeps coming up in forums and clinic visits, and the answer is less straightforward than a food label.

What the human studies show

Observational studies that compare soda intake with semen analysis results have produced mixed findings. Some cohorts report lower total motile sperm count or a higher percentage of abnormal forms among men with the heaviest intake of artificially sweetened drinks. Other cohorts find no meaningful difference after adjusting for body mass index and overall diet quality.

The randomized controlled trials that would settle this have not been done. That leaves association only. A man who drinks a lot of diet soda is also more likely to carry extra weight, eat fewer whole foods, or have shifted to diet drinks after a health scare. The sweetener may be along for the ride, not driving the result.

Why the diet soda drinker is hard to study

Men who choose diet beverages often do so because they are trying to manage weight or blood sugar. Excess body fat itself affects male hormones through aromatase, an enzyme stored in fat tissue that converts testosterone into estradiol. Higher body fat also raises scrotal temperature through more sitting, tighter clothing, and less physical activity. Those factors have stronger, more consistent links to lower sperm quality than any artificial sweetener does.

When a study finds worse sperm parameters in heavy diet soda drinkers, researchers cannot easily separate the can of soda from the extra 25 kilograms, the sedentary job, and the vending machine diet that often travels with it.

What animal data tell us

Rodent studies have shown that very high doses of aspartame can increase oxidative stress in testicular tissue and produce abnormal sperm morphology. Saccharin and sucralose have triggered similar changes in some animal models. Those doses are far above what a human gets from a can or two each day. The animal findings are useful because they tell us these compounds are not biologically inert. They do not tell us what a realistic human dose does to sperm over years.

Possible mechanisms in the body

Artificial sweeteners can alter the gut microbiome in some people, and the gut talks directly to the immune system and metabolic signaling. Aspartame breaks down into phenylalanine, aspartic acid, and methanol. At very high exposure, methanol converts to formaldehyde. The average person metabolizes the small amounts from diet drinks without issue, but the pathway exists.

The most plausible human mechanism is indirect. If a man replaces sugary drinks with artificially sweetened ones but keeps eating ultra-processed food, his overall dietary pattern remains poor. That pattern, with or without sweeteners, is the bigger fertility risk.

The packaging confound

Most artificially sweetened drinks come in plastic bottles or aluminum cans with plastic linings. Those linings can contain bisphenols, including BPA and its chemical relatives. BPA and related compounds are known endocrine disruptors, and human studies have associated higher bisphenol exposure with lower sperm concentration and motility. Diet drinks that sit in hot trucks or warehouses may absorb more from the liner over time. Almost no study of diet soda and fertility measures the container, so the drink and the packaging get lumped together.

What to do with this

If you are working to improve fertility, artificial sweeteners are not the first factor to change. The following have far stronger evidence behind them:

  • Sleep quantity and consistency
  • Regular exercise, without overtraining
  • Alcohol intake and smoking
  • Body weight and body fat percentage
  • Heat exposure to the testicles from prolonged sitting or tight underwear

That said, replacing diet drinks with water, sparkling water with lemon, or unsweetened tea costs nothing and removes one source of uncertainty. If you drink four or more artificially sweetened drinks a day, cutting back to one or two and replacing the rest with water is a reasonable move. If you have an occasional diet soda with a meal, the current evidence does not justify losing sleep over it. The men's health decisions that move the needle are the daily ones, not the single can.
