What a standard semen analysis actually measures
A proper semen analysis includes sperm parameters, but it also includes “whole sample” measurements that can change interpretation.
1) Whole-sample measures (the environment)
- Volume: Low volume can come from incomplete collection, short abstinence, retrograde ejaculation, or obstruction. It is not automatically a sperm production issue.
- Liquefaction time and viscosity: Semen is supposed to liquefy. If it stays thick, motility can look worse and the sample can be harder to interpret. Sometimes this pattern is seen with inflammation or prostate and seminal vesicle factors.
- pH: The chemical environment matters for sperm function. Unusual pH values can be a clue when paired with other findings.

- Sperm concentration: How many sperm per milliliter.
- Total sperm number: Concentration multiplied by volume (this one gets overlooked, and it should not).
- Motility: How many move, and sometimes how many move forward effectively (progressive motility).
- Morphology: Shape, often graded with strict criteria.
- Vitality (sometimes included): Useful when motility is low, because non-moving sperm might still be alive.
3) Cells and inflammation markers (varies by lab)
- Round cells and white blood cells: These can raise the possibility of inflammation, but they are easy to overread without confirmatory testing and clinical context.
What “sperm analysis” might mean when someone orders it
In the real world, “sperm analysis” can mean three different things. This is where men get tripped up.
- Shorthand for semen analysis: Common in casual conversation, sometimes even in clinic portals.
- A narrower sperm-only screen: Some services focus mainly on concentration and motility (and sometimes morphology) and do not fully report viscosity, liquefaction, pH, or cell counts.
- A package that includes add-on sperm tests: Often in fertility clinics, “sperm testing” can include specialized tests like sperm DNA fragmentation.
If you are unsure what you had done, ask for the actual report and look for items like volume, pH, and liquefaction. If those are missing, you likely did not get a full semen analysis.
The underused angle: semen analysis is also a “gland function” check
Most men think a semen analysis is only grading sperm. Clinically, it can also act like a first-pass look at the plumbing and the glands that make semen.
Here are a few patterns clinicians pay attention to:
- Normal concentration with low volume: That can point toward collection issues or problems with semen fluid contribution rather than sperm production alone.
- Low motility with high viscosity: Sometimes sperm are not “lazy,” they are stuck in a thick sample. That changes what you suspect and what you do next.
- Odd pH paired with other abnormalities: This can add weight to concerns about obstruction or gland contribution, depending on the rest of the picture.
This is why two men can have the same sperm concentration and very different clinical interpretations based on the rest of the semen profile.
WHO reference ranges are not a pass-fail exam
Most labs compare results to WHO reference limits. That is useful, but it is easy to misunderstand.
- Above the reference limit does not guarantee fertility. Fertility is a couple-level outcome, not a single-number outcome.
- Below the reference limit does not automatically mean infertility. It means follow-up and context matter.

How to get a result you can actually trust

- Do not ignore recent illness. Fever can temporarily impair sperm production, and its effects can show up weeks later. Tell the lab or your clinician if you were sick recently.
- Collect the sample correctly. Avoid typical lubricants, aim for complete collection, and follow timing and transport instructions closely if collecting at home.

When a semen analysis is “normal” but pregnancy still is not happening
This is where men either get dismissed or sold expensive add-ons without a clear reason. Both are mistakes.
In select situations, clinicians consider additional sperm-focused testing. One of the most discussed is sperm DNA fragmentation (SDF).
