Walk into any large bookstore and find the fertility aisle. Most run forty books deep, and thirty-six of them are addressed to women. The four left for men include a reprint from the 1990s and a workbook you will never fill in. That shelf is a fossil record of how late reproductive medicine started paying attention to the male half of conception.
The usable books on male fertility exist. They were just never filed together. You have to pull them from four different shelves: evolutionary biology, environmental epidemiology, endocrinology, and the small sliver of actual male fertility books. Reading them as a stack, in roughly the order the science developed, gives you a clearer picture of the field than any single title.
Why the shelf looks like that
Reproductive medicine spent most of the twentieth century treating male factor as a secondary diagnostic. The World Health Organization did not publish its first standardized manual for human semen analysis until 1980. Before that, there was no common method for counting and grading sperm across labs. Sperm parameters decline gradually with age, and paternal age carries consequences. Fisch walks through what a semen analysis measures, what varicocele is, and why testicular temperature matters.
Parts of the book read dated now. The supplement talk especially. But the core correction was right, and the data behind it has only gotten stronger. Men get older. So does their sperm. Research on paternal age has linked older fathers to a higher likelihood of several conditions in offspring, and the effect runs on a gradient rather than a cliff. There is no switch that turns off at forty. The system degrades slowly over decades.
The second correction: fertility becomes an environmental health story
Count Down by Shanna Swan, published in 2021, is the book that turned male fertility into an environmental health story. Swan is an epidemiologist who worked on sperm count decline research. The headline data point comes from meta-analyses led by Hagai Levine and colleagues. The original 2017 analysis found sperm concentration among men in North America, Europe, Australia, and New Zealand fell by more than half between 1973 and 2011. The 2023 update in Human Reproduction Update expanded the data set and found the decline continuing across more countries.
Swan's contribution is the proposed mechanism. She traces the decline to phthalates, bisphenols, pesticides, and other chemicals that can interfere with testosterone production during early development. The book's weakness is that some of those connections remain associations in humans, even when they are well established in animal models. Swan flags this more often than her critics give her credit for, but the book still runs ahead of the human evidence. Read it for the map, not the final word.
Estrogeneration by Anthony G. Jay, published in 2016, reads like the household-level companion to Swan. Jay is a biochemist, and the book walks through the estrogen-mimicking chemistry in plastics, food packaging, and personal care products. He names specific ingredients, which makes the book practical in a way the epidemiology shelves rarely are. The tradeoff is that Jay leans on mechanistic and animal data. Mechanism matters, but it does not equal human outcome. Read this one after Swan and hold the two side by side.
The newest chapter in this environmental story is not in a book yet. Researchers detected microplastics in human testicular tissue and semen, with some cohorts showing them in every sample examined. Hu and colleagues reported the finding in Toxicological Sciences in 2024. Zhao and colleagues published similar detection work in Science of the Total Environment in 2023, as did Li and colleagues in the same journal in 2024. Presence in tissue is established. Whether that presence damages fertility is still an open question. The human data so far show association rather than proven cause. Publishers have not caught up with that nuance, which is a good reason to check the study dates in any fertility book you pick up.
The mechanism books on temperature and sperm
Eve by Cat Bohannon, published in 2023, is not a fertility book. It is an evolutionary history of the female body. But its clearest passage on male reproduction explains why the testicles sit outside the body in the first place. Sperm production requires a temperature a few degrees below core body temperature, and the scrotum, the cremaster muscle, and the whole architecture exist to shed heat. When external heat overwhelms that system, sperm parameters drop.
The research backs this up. Garolla and colleagues documented in Human Reproduction in 2013 that sauna-level heat temporarily lowers sperm count and can worsen morphology, and the effect reverses once exposure stops. Shefi et al. in Fertility and Sterility in 2007 showed semen parameters recovering after men stopped regular wet heat exposure. Minguez-Alarcon and colleagues reported in Human Reproduction in 2018 that men who reported wearing looser, boxer-style underwear had higher sperm concentration and total count than men in tighter underwear. Temperature is one of the few variables in this research you can measure directly. The studies do.
How We Do It by Robert Martin, published in 2013, adds the comparative biology view. Martin is a biological anthropologist, and he shows that human sperm are unremarkable by primate standards and that human male fertility is unusually sensitive to environmental conditions. It is a useful counterweight to the performance-manual tone of the men's health shelf.

Then stop reading and get the basics measured. A semen analysis takes an afternoon. Blood work takes a morning. A doctor can interpret both in the context of your own history. Some men start with those two data points before changing anything else, and that is a reasonable order. Individual situations vary a lot, and none of these books is a diagnosis.
