Most men don’t connect a missing morning erection to the possibility that their sperm count is dropping. But the same hormones that trigger that morning signal are the ones required for your testicles to make sperm. When those hormone levels shift, the symptoms often show up in the bedroom first, even before a semen analysis picks up the problem.
The European Male Aging Study followed more than 3,000 men and identified three symptoms that track most closely with a clinically low testosterone level: poor morning erections, low sexual desire, and erectile dysfunction (Wu et al., New England Journal of Medicine, 2010). None of those symptoms announces itself as “your hormonal axis is off.” They just feel like a gradual fade; that is exactly why they are worth paying attention to.

Low Testosterone
Testosterone is the headline hormone in any conversation about male fertility, and for good reason. The early signs are not always the ones you see listed on a men’s health poster. The three sexual symptoms flagged in the EMAS study are the most reliable, but you might also notice:

The estradiol number matters just as much as the testosterone one.
High Prolactin
Prolactin is the hormone most people associate with breastfeeding, but it also has a direct suppressive effect on the hypothalamic-pituitary-gonadal axis in men. When prolactin climbs too high, it dampens the release of gonadotropin-releasing hormone (GnRH), which lowers LH and follicle-stimulating hormone (FSH), the two pituitary hormones that tell your testicles to produce testosterone and sperm.
The early red flags are almost always sexual: a pronounced drop in libido that does not track with stress or relationship trouble, difficulty getting or keeping an erection even when desire is present, and in some cases a cloudy or milky discharge from the nipples. Moderate hyperprolactinemia can exist for months before a man notices any other physical change, which is why endocrinologists tend to test prolactin as part of any workup for unexplained sexual dysfunction.
A 2003 study by De Rosa and colleagues in the Journal of Endocrinological Investigation documented that treating elevated prolactin levels in infertile men with a dopamine agonist normalized several sperm parameters once the hormone was brought back into range. The key point: the disruption is centrally driven, so the sperm quality drop can reverse when the prolactin source is addressed.
Thyroid Dysfunction
The thyroid gland and male fertility do not seem like obvious bedfellows, but the connection runs through SHBG. An underactive thyroid (hypothyroidism) tends to depress SHBG, which can lead to a lower total testosterone measurement while free testosterone squeaks by in the normal range. An overactive thyroid (hyperthyroidism) pushes SHBG high, which can bind up testosterone so tightly that free testosterone plummets despite a normal total number. Both scenarios can impair sperm maturation and motility.
Early signs of thyroid trouble are frustratingly nonspecific on their own. You might feel cold when others are comfortable, or hot and sweaty at rest. Your resting heart rate may shift, bowel habits change, and your energy level can swing. Hair loss from the outer third of the eyebrows is a classic finding in hypothyroidism, though not foolproof. What makes the thyroid worth checking early in a fertility workup is that the hormonal disruption often predates obvious changes in weight or heart rate. A 2011 review by Rajender and colleagues in Frontiers in Bioscience concluded that thyroid hormone receptors are present on Sertoli cells and developing germ cells in the testis, meaning the thyroid can affect spermatogenesis directly, not just through SHBG alterations.
Chronic Stress and Cortisol
Cortisol is not an enemy. It gets you out of bed and helps you handle an acute threat. The problem arises when stress becomes chronic and the daily rhythm of cortisol flattens out. High evening cortisol, in particular, suppresses the pituitary’s release of LH, which reduces testosterone production in the testicles. The mechanism is well-described: glucocorticoid receptors in the Leydig cells respond to cortisol by downregulating the enzymes involved in steroidogenesis (Whirledge and Cidlowski, Trends in Endocrinology & Metabolism, 2013).
The early signs of a cortisol-driven hormonal imbalance are a mix of the physical and the cognitive. You may notice you wake up tired even after a full night’s sleep, or that you suddenly carry more fat around your midsection while your arms and legs stay lean. Your libido dips, and you become less resilient to small daily stressors. Some men report a thinning of the skin or slower wound healing.
Fertility research has connected high perceived stress and elevated cortisol to lower sperm concentration, reduced progressive motility, and increased sperm DNA fragmentation. A 2014 study in Fertility and Sterility found that men with higher salivary cortisol levels had a significantly lower probability of conception during a 12-month window. The cortisol effect compounds if poor sleep is part of the picture because much of a man’s daily testosterone release happens during deep, slow-wave sleep.
Putting the Clues Together
Individually, any of these signs can be dismissed as a bad month. A few nights of poor sleep. A tough project at work. The reason they matter collectively is that fertility is a long game. Sperm production takes about 74 days from start to finish. A hormonal imbalance that begins today will not appear in a semen analysis for more than two months, but the bedside signals often arrive within weeks.

Testosterone boosters sold over the counter do not raise testosterone in any reproducible way in placebo-controlled studies, and they can introduce ingredients that affect liver function or blood pressure. If a hormonal problem exists, the first job is to figure out whether the driver is testicular failure, a pituitary issue, a thyroid problem, or a lifestyle factor that can be reversed.
Lifestyle adjustments are not a substitute for medical workup, but they can help nudge some numbers in the right direction. Sleep matters enormously: a study in JAMA in 2011 showed that one week of sleep restriction to five hours per night dropped daytime testosterone in healthy young men by 10 to 15 percent. Excess body fat ramps up aromatase activity, so dropping even a modest amount of visceral fat can lower the estradiol-to-testosterone ratio. Heavy alcohol intake raises prolactin and directly damages Leydig cells. None of this is magic. It is just giving the system less interference.
